Leverage CGM Technology in Your Practice With the New Personal CGM Playbook

Kelly A. Brock, PharmD, RPh, Diana Isaacs, PharmD, BCPS, BCACP, BC-ADM, CDCES, FADCES, and Patricia L. Scalzo, MSN, NP, RN, CDCES, have joined together to share their collective expertise as contributors for the Personal CGM Playbook. The goal was to create something that was informative, practical, accessible and easy to implement into practice. This playbook was created with the care team in mind!

Personal continuous glucose monitoring can be a life-changing tool for someone with diabetes and has become the gold standard for those on an intensive insulin program. Unfortunately, many people with diabetes that could benefit from the use of this technology, do not currently have access to it.


There is a step-by-step practical pathway to help you and your practice get started with personal CGM.


The Personal CGM Playbook provides a background into CGM technology, an overview of the evidence surrounding CGM, advantages and disadvantages of CGM, candidate selection and payor considerations. Patient and provider barriers are addressed and possible solutions are identified. There is a step-by-step practical pathway to help you and your practice get started with personal CGM.

Role of the Pharmacist

A pharmacist can utilize this playbook to help people with diabetes embrace personal CGM technology, which will provide valuable information to manage their diabetes. As healthcare providers with medication expertise, pharmacists should be considered vital members of the healthcare team involved in implementing personal CGM. Pharmacists are well-positioned to collaborate with the person with diabetes and their healthcare team to monitor medication therapy and make recommendations for adjustments in therapy as needed to maximize outcomes for people with diabetes.

Role of the Diabetes Care and Education Specialist

The diabetes care and education specialist can utilize this playbook to leverage CGM technology into their practice. The step-by-step approach that is laid out in the playbook makes it easy to implement personal CGM in any setting.

This puts the following ADCES Project vision pillars into action:

  • Serve as the epicenter of the diabetes care team, integrating the clinical and self-management aspects of care.
  • Embrace technology and data interpretation to ensure safe, real time and quality care and education for people with diabetes and the diabetes care team.
  • Embrace and guarantee person-centered care.
