Description
This certificate program is intended for complementary workers who have a defined role in a certified or recognized diabetes education or prevention program. They may also be aligned with practices that serve a dedicated or focused proportion of people with diabetes. The content for this program is based upon the skills and knowledge defined for the Diabetes Community Care Coordinator.

Coursework
This certificate program is a self-study, self-paced program. There are approximately 11 hours of coursework. There is no CE/CME credit for this activity, but learners will earn a certificate of completion upon successful completion of all components.

Intended Audience
This activity is designed for members of the diabetes care team focused on linking individuals to resources, building relationships, and supporting people with, or affected by, or at risk for diabetes and cardiometabolic conditions.
This includes, but is not limited to: community health workers (CHW), health educators/coaches, medical assistants (MA), certified nursing assistants (CNA), licensed practical nurses (LPN), registered nutrition/dietetic technicians (NDTR), military medics and corpsmen, pharmacy assistants/technicians, physical therapy assistants, nutritionists, dental hygienists, emergency medical technicians (EMT), and other similar roles.
Access Period
You have TWO (2) months from the date of your program registration to complete all program activities, submit the final program evaluation, and claim your certificate of program completion.
Certificate of Completion
There is no CE/CME credit for this activity. Upon completion of the program, you will receive a Certificate of Program Completion. The certificate provides recognition for your continuing education efforts and demonstrates your initiative in acquiring additional training in diabetes self-management education.
This is a certificate program, not a certification. Generally, a certificate program provides training in a specific body of knowledge and/or skill set. Successful completion of a certificate program provides a certificate of completion, not a credential. A certificate program generally does not require renewal or CE to maintain the certificate.
