Description
This comprehensive Inpatient Certificate program is an asynchronous program designed to equip healthcare professionals with a specific focus on the nursing role, with the knowledge and skills necessary for effective inpatient diabetes care. The program covers essential aspects of diabetes management, from initial patient assessment, glucose targets based on diagnosis and treatments, nutrition therapy, to medications, including insulin, oral, and non-insulin injectable agents. Participants will learn how to prevent and manage acute complications, utilize diabetes technology safely, and ensure smooth transitions of care for patients upon discharge. Emphasis is placed on respectful, inclusive, and person-centered communication, addressing social determinants of health, and supporting patients' self-management capabilities.
Learning Objectives
Upon successful completion of this program, participants will:
- Enhanced Clinical Competence: Demonstrate a comprehensive understanding of inpatient diabetes management, including assessment, treatment, and coordination of care, resulting in improved patient outcomes and safety.
- Improved Communication Skills: Utilize respectful, inclusive, and person-centered communication strategies to interact with patients effectively.
- Effective Use of Medical Nutrition Therapy: Apply evidence-based medical nutrition therapy principles to support glucose management, metabolic needs, and preparation for discharge.
- Addressing Social Determinants of Health (SDoH): Recognize and address SDOH that may affect diabetes management and require tailoring of care plans to meet the diverse patient needs. .
- Skills for appropriate Insulin and Medication Management: Summarize safe use of insulin therapy and oral/non-insulin injectable agents, ensuring appropriate usage, monitoring, and adjustments, particularly in complex clinical scenarios such as steroid therapy.
- Prevention and Management of Complications: Identify, prevent, and manage acute complications such as hypoglycemia and hyperglycemia, utilizing best practices to ensure patient safety and mitigate risks during hospital stays.
- Integration of Diabetes Technology: Assess and monitor use of diabetes technology, such as continuous glucose monitoring (CGM) and insulin pumps ensuring adherence to clinical guidelines, internal policies, and regulatory requirements.
- Facilitating Safe Transitions of Care: Coordinate and support safe transitions of care, ensuring continuity of care and patient/family readiness with the necessary diabetes self-management skills and resources.
