Description

Spanish-speaking participants in the National Diabetes Prevention Program need support to make and sustain changes that profoundly improve, reduce risks of, and even reverse chronic disease. This session describes a partnership with the Black Women’s Health Imperative and supported by CDC’s 1705 cooperative agreement that embeds in-person lifestyle change programs in churches serving Hispanic participants. The presenter will share how they enrolled and retained participants and will give Insights into delivering the program in faith-based settings. Finally, attendees will learn about outcomes and additional work planned to expand enrollment and retention.

Learning Outcome

Attendees will apply evidence-based solutions and interventions for equitable healthcare, including comprehensive care for individuals with diabetes and related conditions, addressing behavioral health concerns, advocating for sustainable technology-enabled care, implementing interprofessional team practices, and enhancing clinical care and support in diverse settings.
