CGM (& Insulin Delivery Device) Affordability Programs

Current manufacturer programs & other resources to get more people living with diabetes the care they need.

There are a number of reasons why people with diabetes can't get the devices they need. One of the largest is cost. And CGMs and insulin pumps can be expensive. If you have a client or patient struggling to pay for supplies given no insurance or inadequate insurance, take a look at the current assistance programs in the drop-down menus below, and don't hesitate to contact the manufacturers for help.

Abbott

Products: FreeStyle Libre 14 Day, Libre 2, Libre 3

MyFreeStyle program 1 free sensor and reader.

Manufacturer Program for people with commercial insurance: $75 for 2 sensors, $65 for reader.

Medicare not eligible for either of the above but can look for discount card programs.

Dexcom

Products: Dexcom G6 & G7

For those without CGM coverage: If a patient does not have CGM coverage or has a high copay, Dexcom offers a pharmacy savings program. Once the patient has a prescription, they can use Dexcom Coupon to save on the cash price of the product (but note, smart devices are sold separately).

Qualifying patients can save $200 or more* with this coupon on:

  • G6 Sensor/Receiver
  • G7 Wearable/Receiver

Dexcom Patient Assistance Program Criteria (from the Dexcom website):

"Dexcom Patient Assistance Program offers discounts to US residents who are at or below an identified poverty line. To be eligible to participate in the Dexcom Patient Assistance Program, you will need to complete an application and provide any requested supporting documentation.
You may be eligible for the Dexcom Patient Assistance Program if:
• The patient is a resident of the United States
• The patient has been diagnosed with Type 1 Diabetes
• The patient has no insurance, or has insurance and is not enrolled in a state or government insurance plan
• The patient is 2 years of age or older
• The patient meets the household income guidelines for the program (currently 400% of the US Federal Poverty Level)"
 

This program may include:

  • Shipments include 3 months of G7 sensors. Dexcom will offer up to 2 shipments of G7 at a reduced cost

Ascensia

Products: Eversense GGM System

Programs for people with commercial insurance or no insurance

Pass Program provides the first sensor/transmitter and second sensor for $99.00
And then ongoing, After the first full year on Eversense E3, out-of-pocket expenses will not exceed $600.00 for the next sensor or sensor/transmitter combination

Medtronic

Products: Guardian 3, Guardian Connect

Programs for people without insurance coverage for Guardian sensors:

Patients may be eligible eligible for a CGM access discount if they meet the following requirements:

  1. The individual does not have insurance coverage for Medtronic CGM
  2. The person uses multiple daily injections or use an insulin pump
  3. They have a valid prescription for your CGM supplies from your healthcare professional

Cost is reduced to $180 per transmitter (12-month warranty) and $60 per box of 5 sensors (7-day wear/sensor)- last confirmed 9/23

Other Medtronic Programs:
If recently lost your job or health insurance, you can receive up to 3 months of sensors at no cost. Defer payments for up to 3 months if you or a family member is laid off or furloughed from job.

Flexible Payment Plan Options

The Tandem Choice Program, available for a limited time, offers the Tandem Mobi System for $199 to customers who purchase a t:slim X2 between July 1, 2023, and December 31, 2023. Those with a t:slim X2 purchased before July 1, 2023, and more than 12 months remaining on their warranty can also join for the original price of $999. Eligible customers can participate until December 31, 2024, and a switch from t:slim X2 to Tandem Mobi is possible upon participation.


Omnipod

Products: Omnipod Dash & Omnipod5

Financial Assistance is Available for:

  • Individuals currently or just starting on the Omnipod DASH® Insulin Management System or Omnipod 5 Automated Insulin Delivery System.
  • Individuals currently on the Omnipod Insulin Management System
  • Individuals who are uninsured, and/or have insurance that does not cover an Omnipod product or cannot afford their insurance out-of-pocket obligations

  • Free 10 day trial of Omnipod 5.

  • Does not apply to those with government insurance.
