Educating Connected Device Wearers on Data Uploads

The review of data from wearables and connected devices is an important part of the telehealth/virtual visit for the person with diabetes (PWD) and their health care provider (HCP). This resource guide provides an overview to assist the person with diabetes to upload their personal data from continuous glucose monitors, connected insulin delivery devices, and insulin pumps in preparation for a visit with their health care provider.

Health care providers and support staff can provide education for users of technology about how to send or give providers access to their data for in-office visits as well as telehealth or virtual care visits. Keep in mind that this may be the very first experience that an individual has had with uploading their data, because clinic staff may have always done this for them in the past at in-person health care visits.

Telehealth visits occur between individuals and health care providers over a chosen audio-visual platform. The appointed staff person should provide education about the platform and about uploading their device data for the person with diabetes prior to the time of their virtual/office visit. This will ensure that the PWD and their health care provider are able to access their data on a supported device whether using a smart phone, tablet, laptop or PC.

 

Roles of the Team Members

Health Care Providers and Support Staff

  • Contact person with diabetes prior to their session provide education about the telehealth process.
  • Review for diabetes device use and provide education about data upload process. 

Person With Diabetes/Technology User

  • Follow instructions for use of telehealth platform.
  • Follow instructions for uploading/submitting data from device to health care provider via app, PC software or fax/email. 

 

Sharing Data by Company & Brand

Dexcom

Patients must give permission to share their CGM data by accepting a sharing invitation from your clinic or initiating the sharing in their Clarity account.

  • Patients or clinics can stop sharing data at any time.
  • Clinics are able to use Dexcom Clarity to invite patients to share data with the clinic.  

Invitations provide a sharing code that patients enter into their personal Dexcom Clarity accounts. Once they enter the code, the accounts will automatically share data between them. 

Inviting a Patient to Share Data Use Dexcom Clarity to invite patients to share data with your clinic. Provide an invitation to patients in print or by email. Invitations provide a sharing code that patients enter on their personal Dexcom Clarity accounts. Once they enter the code, the accounts will automatically share data between them. 

To create an invitation: 

1. Click Patients. 
2. Click the patient’s name you want to invite. 
3. Click Share data. 
4. Click Print an Invitation to view and print, or Email an Invitation, then enter the patients email address. 

5. Click Invite, then Close

Sharing Continuous Glucose data from a Smartphone using the Dexcom CLARITY app

Watch this video to learn about using the Dexcom CLARITY account on PWD phone. This will allow direct link of PWD Dexcom CLARITY account to the clinic account. Sensor readings will be available to health care provider for review. The Dexcom CLARITY app also allows View, Save, Print and Email of reports via PDF creation.

Senseonics Eversense

Eversense - How to share sensor readings with health care provider

Eversense CGM consists of: Sensor, Transmitter, Mobile CGM app, and web-based Data Management System Data Management System (DMS). 

Watch this video to learn about Eversense CGM.

Eversense CGM can wirelessly upload data via the mobile app to DMS. You can also use your computer to upload data to DMS by connecting the smart transmitter to your computer by USB cable and use of the DMS client application.

Steps Overview:

  1. Prior action: Install Eversense mobile app and create account. This would be done when sensor/transmitter was placed.
  2. Sign in to the Eversense DMS web app with account created for mobile app.
  3. Turn on Auto synch in Eversense mobile app so data is pushed to Eversense DMS every 2 hours.
  4. From DMS Data Share menu option: Data can be shared with health care provider by linking to your DMS account to clinics Eversense DMS pro account. Clinic will provide their Pro account number.

How to share sensor readings with health care provider

Carelink Personal Software is used by people with diabetes to share their data with the health care provider. The Carelink System Therapy Management Software is used by the health care provider to receive the data from people with diabetes. 

The person with diabetes can view this video How to Use Carelink Software to set up their personal account, link it to provider clinics, learn how to upload devices, and get information about the views and reports available to understand their CGM data.

Directions for the person with diabetes on how to use Carelink Personal Software to share data with the provider:

  1. Create an account in the Carelink Personal Software Portal 
  2. Upload CareLink data through MiniMed™ Mobile app’s Sync to CareLink feature or the Blue Adapter.
  3. Review reports, such as the Assessment & Progress Report, to understand trends.
  4. Discuss results with your health care team and develop your individual health plan.
